Starting VELETRI^®

Vial(s) of VELETRI^® (epoprostenol) for Injection 0.5 mg or 1.5 mg powder

The treatment of pulmonary arterial hypertension with intravenous (IV) epoprostenol, including VELETRI^®, can seem complicated and frightening at first. Over time, for many people, it becomes more routine and easier to manage.

To start VELETRI^®, you will need to undergo a surgical procedure to insert a permanent catheter (very thin tube) into a large vein near your heart. A small, battery-operated, portable pump that’s attached to a cassette will supply VELETRI^® continuously from the other side of the catheter.¹

Once your catheter is in place, you will continue taking VELETRI^® and your portable infusion pump will control the rate of medication you receive. Once therapy with VELETRI^® begins, it must continue uninterrupted, 24 hours a day, 7 days a week.¹ Do not lower your dose of VELETRI^® or stop taking it unless your healthcare professional tells you to. All dose changes must be closely monitored by your healthcare professional.

Preparing and Storing VELETRI^®

VELETRI^® is supplied as a powder in small, sterile vials. When unopened and stored at room temperature (68°F to 77°F/20°C to 25°C), vials of VELETRI^® can be used until the date printed on the package. The unopened vial should be kept in the carton and not exposed to direct sunlight. Each vial is for single use only; discard any unused solution.¹

Before VELETRI^® can be used, it must first be mixed (reconstituted) and then further diluted with either Sterile Water for Injection, USP, or Sodium Chloride 0.9% Injection, USP. Your specialty pharmacy will send all the necessary supplies needed for preparation.¹

When you start VELETRI^® therapy, you have the option to either:

Prepare and use your medicine immediately for up to 24 hours at room temperature (77°F/25°C) at any concentration without ice packs, or
Prepare and store fully diluted cassettes in the refrigerator (36°F to 46°F/2°C to 8°C) for up to 8 days and then administer at room temperature (77°F/25°C) for up to 24 hours, without ice packs.

In both cases, your healthcare professional will work closely with you and give you specific directions for preparing, storing, and using your medicine. Never change the way you prepare or store VELETRI^® unless instructed to do so by your healthcare professional.

Using VELETRI^®

It is recommended that a single cassette not be administered by pump beyond 24 hours
You can store a backup cassette for 24 hours at room temperature (77°F/25°C) without ice pack
You may administer VELETRI^® without ice packs and remain on the same 24-hour cassette

Short excursions at 104°F/40°C are permitted for up to:

2 hours for concentrations
below 15,000 ng/mL

4 hours for concentrations between 15,000 ng/mL and 60,000 ng/mL

8 hours for concentrations
above 60,000 ng/mL

Helpful Tips – At a Glance

Wearing your pump

A portable, battery-operated pump administers VELETRI^®
To wear this pump, you may attach it to a belt around the waist or carry it in a purse or shoulder bag

Developing an emergency plan

VELETRI^® must be infused continuously via a central venous catheter using an ambulatory infusion pump. If there is a disruption of delivery of VELETRI^®, call 9-1-1 immediately

If the catheter to the pump suddenly disconnects, immediately reconnect and call your healthcare professional. Tell your doctor that your dosage has been interrupted and he or she will provide instructions on what to do next

Ask your nurse about talking with your local emergency medical services (EMS)
Prepare an emergency supply kit and take it with you whenever you leave home. Put all of your supplies and medication in a bag you can carry with you, so it cannot get lost. Many hospitals do not normally stock the medication or special supplies needed to administer your therapy. Being prepared can help you avoid many problems.

Involving others in your care and treatment

Build a support network that includes family members, caregivers, other people with PAH, and members of your healthcare team (doctors, nurses, specialty pharmacies)
Involve a family member or caregiver when preparing your VELETRI^® so that they will also know how to prepare your medicine
Reach out to these individuals for education and support
Always talk to your doctor or healthcare team about issues or questions you may have about your condition or treatment, particularly:

If you are about to start on any new medicines
If you become pregnant or are planning to become pregnant
If you have not used your medicine exactly as prescribed
If you experience any side effects or allergic reactions while taking VELETRI^®
